Referral form

Thank you for expressing interest in receiving support from Firstchance. 

Firstchance provides early intervention and school-aged support to children aged 0-12 years however, we only accept referrals for children aged 0-9 years.

Please complete our online referral form and we will be in touch with you as soon as possible.

We currently have vacancies for Occupational Therapy. 

Whilst you are waiting to hear back from us, why not follow us on our Facebook or Instagram page. 

What you will require before you start:

  • Your child's NDIS participant number (if one has been issued)
  • Electronic copies of any relevant reports (scanned copies or images are ok)
  • Details or any current service providers
  • If you are referring on behalf of a family you will be required to know sibling details (name, DOB, gender)
